Europe is ageing rapidly and average life expectancy is increasing year on year. Making those later years of life as healthy and active as possible represents a huge challenge, both for individuals and policy makers. Nevertheless, aging is often accompanied by losses in various areas: loss of physical or cognitive functions, loss of well-being, reduced social interactions, etc. If such losses are not noticed and taken care of in time, the quality of life of the elderly (and their families and environment) deteriorates significantly. In the process, pathologies and limitations increase, weakening the elderly person. Although various support systems already exist (such as home care, welfare care, etc.), early, efficient and systematic detection of frailty on the one hand and active guidance towards empowerment and appropriate support within the older person's environment on the other is lacking. To address this, this project aims to develop a support pathway for frail elderly people and their environment.
