Decubituspreventie bij volwassen intensievezorgenpatiënten

Decubitusletsels (doorligwonden) zijn een frequente hospitalisatiecomplicatie, geassocieerd met nefaste fysieke en psychische gevolgen, en substantiële meerkosten voor gezondheidszorg.
Ondanks het uitgesproken risicoprofiel van intensievezorgen(IZ)patiënten is decubitus bij deze populatie een weinig geëxploreerd probleem.
Van talrijke factoren die geassocieerd zijn met kritieke ziekte is de onafhankelijke invloed op het ontstaan van decubitus nog ongekend, waardoor valide risicopredictiemodellen heden ontbreken. Gebrek aan kwaliteitsvolle grootschalige data verhindert echter dieperliggende inzichten te verwerven en accurate risicoschalen te ontwikkelen. Daarom voerden we op 15/5/2018 een wereldwijde prospectieve puntprevalentiemeting van decubitus bij volwassen IZ-patiënten, resulterend in 13.311 geïncludeerde patiënten (1.167 IZ-afdelingen - 90 landen - alle continenten vertegenwoordigd).

Doelstellingen
Via de gecollecteerde gegevens:
(a) populatiespecifieke risicopredictiemodellen ontwikkelen, en hun pilot-implementatie evalueren;
(b) duurzame positieve wijzigingen bekomen in attitudes, kennis en praktijkvoering bij IZ-clinici wereldwijd;
(c) het draagvlak voor patiënten-/familieparticipatie in decubituspreventie objectiveren.

Verwachte impact
De risicopredictiemodellen worden verwacht significant bij te dragen tot reductie van de prevalentie, tot een meer onderbouwd, efficiënt, effectief en duurzaam gebruik van (schaarse) geldelijke en materiële resources, en een toename van kwaliteit en veiligheid van de zorgprocessen op IZ.
Wereldwijde sensibilisering/educatiecampagnes worden verwacht duurzame positieve attitudes, betere kennis en onderbouwde wijzigingen in de praktijkvoering qua decubituspreventie te creëren, en vervolgonderzoek te initiëren. Identificatie van het draagvlak voor patiënten-/familieparticipatie zal aantonen of hiervoor toekomst is in (Vlaamse) zorginstellingen.

Decubitus prevention in adult intensive care patients

Decubitus injuries (bedsores) are a frequent hospitalization complication, associated with pernicious physical and psychological consequences, and substantial additional healthcare costs.
Despite the distinct risk profile of intensive care (IZ) patients, pressure ulcers are a little-explored problem in this population.
Of numerous factors associated with critical illness, the independent influence on the development of pressure ulcers is still unknown, thus valid risk prediction models are lacking today. However, lack of quality large-scale data prevents deeper insights and development of accurate risk scales. Therefore, we conducted a global prospective point prevalence measurement of pressure ulcers in adult IZ patients on 15/5/2018, resulting in 13,311 included patients (1,167 IZ units - 90 countries - all continents represented).
Objectives
Through the collected data:
(a) develop population-specific risk prediction models, and evaluate their pilot implementation;
(b) obtain lasting positive changes in attitudes, knowledge and practice among IZ clinicians worldwide;
(c) objectify support for patient/family participation in pressure ulcer prevention.

Expected impact.
The risk prediction models are expected to contribute significantly to prevalence reduction, to a more informed, efficient, effective and sustainable use of (scarce) monetary and material resources, and to an increase in quality and safety of care processes at IZ.
Global awareness/education campaigns are expected to create sustainable positive attitudes, improved knowledge and informed changes in practice in terms of pressure ulcer prevention, and initiate follow-up research. Identification of support for patient/family participation will show whether there is a future for this in (Flemish) healthcare institutions.
